Instructions for filling
GNM application form – 2024
- Before filling application form, candidates are suggested to keep all the required documents / certificates ready such as educational certificates, photograph, signature etc.
- Candidates are required to have a valid email ID and mobile number which should be kept active throughout the application process.
- Instructions for uploading document/photo –
- : PHOTO – A front facing passport size photograph of candidate, without cap/hat & sun-glasses . Image should be of 165 x 125 pixels in jpeg/jpg/png format and should not exceed 200 KB. Photograph background should be white.
- : SIGNATURE – A scanned image of candidate’s signature with white background. Image should be of 80×125 pixels in jpeg/jpg/png format only and should not exceed 100 KB.
- : DOCUMENT/CERTIFICATES – A scanned copy of 10th & 12th mark sheet/certificate, sponsorship letter (if applicable) in PDF format only, should be uploaded. File size should not exceed 200 KB.
- Instructions for making online payment –
- Open UPI App (Ex. Phonepe, Google-pay, Paytm).
- Click on “TO BANK / UPI ID”.
- Click on “+” sign to select “PAYEE BANK”
- Click on “Bank Accounts” then Add Payee/Bank Account.
- Select Bank (AXIS BANK) from list.
- Enter Account Number (The account number mentioned as mentioned in notification)
- Re-Enter Account Number (Same as above)
- Enter Account Holder Name (As mentioned in notification)
- Proceed to pay
- : Application fee for filling GNM application form including prospectus is Rs. 800/- (INR).
- : Application fee amount should be paid in a single transaction.
- : Please be very care-full while making the payment and entering the UTR Number / UPI Transaction id, as your payment will be manually verified though the UTR Number / UPI Transaction id.
- : In the event of wrong / false or no UTR Number / UPI Transaction id entered or left blank, no further correction shall be entertained and the candidature-ship shall be cancelled/terminated.
- : In the event of same UTR Number / UPI Transaction id entered in more than one application form, all the applications involved in the event shall be cancelled.
- : In the event of payment failure / wrong deductions, SON The Duncan Hospital is not liable to pay / repay in such cases. Hence it is suggested to make any financial transaction at your own risk.
- After successful payment, candidate should start filling GNM applications form.(Please keep UTR Number / UPI Transaction id ready before start filling form).
- Application fee once paid shall neither be adjusted nor refunded in any case.
- Before filling the form, a quick basic eligibility check shall be done to confirm your eligibility.
- This form is divided into 5 (Five) steps as follows.

- Educational Information.
- General Information.
- Document upload
- Final Submission.
- After successful payment & form submission, candidate shall receive a confirmation email on the email provided. If not wait for 3 working days or write to us at it.duncan@eha-health.org
- Once scrutiny of forms is completed, hall tickets for appearing in PNT (Pre-Nursing Test) will be sent to candidates on provided email addresses.
- Note – All the candidates will have to appear for “PRE-NURSING TEST” at
